OVERVIEW
Apicoectomy is an oral surgical procedure performed within endodontics and oral surgery. Its clinical objective is to eliminate persistent infection in the periapical tissue surrounding the tip of a tooth root while preserving natural dentition. By removing the apical 3 millimeters of the root—where intricate anatomical branching commonly harbors bacterial biofilms—and applying a fluid-tight seal, clinicians prevent bacterial leakage into surrounding alveolar jawbone (European Society of Endodontology, 2021).
PROCEDURE
1. Local Anesthesia: Injection of local anesthetic containing epinephrine for nerve block and surgical site bleeding control.
2. Surgical Flap Elevation: Incision of the gingival tissue to raise a mucoperiosteal flap and expose the underlying alveolar bone.
3. Osteotomy: Creation of a small access window in the cortical bone over the target root apex.
4. Apical Resection: Resection of the distal 3 mm of the root tip at a 90-degree angle to the long axis of the root.
5. Periapical Curettage: Removal of infected granulation tissue and cystic lining surrounding the apex.
6. Retrograde Ultrasonic Preparation: Cleaning a 3 mm deep root-end cavity along the long axis of the root canal.
7. Root-End Retrofill: Placement of a biocompatible hydraulic calcium silicate cement (e.g., MTA) into the cavity.
8. Flap Closure: Repositioning and suturing of the mucosal flap using micro-sutures.

- Days 3–7: Soft tissue swelling decreases; initial mucosal incision healing takes place. Sutures are evaluated or removed around day 7 to 10.
- Weeks 2–4: Mucosal tissues complete primary healing, allowing patients to resume standard oral hygiene practices and normal eating habits.
- Months 6–12: Radiographic follow-up tracks bone remineralization and structural repair in the periapical region (American Association of Endodontists, 2020).
WHAT WE TREAT

PREPARATION

2. Medical Assessment: Reviewing medical history, systemic conditions, active medications, and blood clotting parameters.
3. Pre-Operative Rinse: Using a 0.12% chlorhexidine gluconate antimicrobial mouthwash prior to surgery.
4. Medication Management: Pre-procedural administration of nonsteroidal anti-inflammatory drugs (NSAIDs) or prophylactic antibiotics as prescribed.
5. Patient Instruction: Arranging post-procedure rest, soft food supplies, and ice packs for immediate home care.
RISKS
Common and Mild: Post-operative pain, localized facial swelling, minor skin or mucosal bruising, transient sensitivity in adjacent teeth.
Uncommon: Localized incisional infection, minor flap dehiscence (wound opening), temporary altered nerve sensation (hypesthesia) in adjacent soft tissues.
Rare and Serious: Permanent neurosensory damage to the inferior alveolar, mental, or nasopalatine nerves; sinus membrane perforation resulting in maxillary sinusitis; complete failure of bone repair requiring tooth extraction.
JOURNEY
The clinical care pathway for an apicoectomy begins with diagnostic workup, including clinical examination and 3D cone-beam computed tomography (CBCT) imaging. On procedure day, local anesthesia is administered, followed by a surgical flap elevation, root tip resection, retrograde canal preparation, and sealing with bioceramic material. Post-operative follow-up includes suture removal at 7 to 14 days and radiographic evaluations at 6 and 12 months to confirm bony healing (American Association of Endodontists, 2020).
