OVERVIEW

Echocardiography is a fundamental diagnostic technique in non-invasive cardiology that utilizes high-frequency ultrasound waves to create real-time images of the heart. The technique allows clinicians to evaluate myocardial contraction, valvular structure and function, pericardial integrity, and intracardiac blood flow patterns through Doppler physics. Modern echocardiographic modalities range from standard transthoracic echocardiography (TTE) in 2D and 3D configurations to semi-invasive transesophageal echocardiography (TEE) and stress-based functional protocols. According to guidelines from the American Society of Echocardiography (ASE) and the European Association of Cardiovascular Imaging (EACVI), echocardiography serves as the primary imaging choice for evaluating suspected or established cardiovascular disease due to its lack of ionizing radiation, safety profile, and high diagnostic resolution.

PROCEDURE

For transthoracic echocardiography (TTE), the patient lies in a left lateral decubitus position. Acoustic gel is applied to the transducer, which is positioned across standard thoracic acoustic windows (parasternal, apical, subcostal, and suprasternal notch). The sonographer acquires standard 2D tomographic planes, M-mode tracings, pulsed-wave, continuous-wave, and color Doppler waveforms. For 3D TTE, matrix-array transducers capture volumetric dynamic frames. For transesophageal echocardiography (TEE), intravenous access is established, intravenous sedation is administered, and the posterior pharynx is anesthetized with topical lidocaine spray. A flexible ultrasound probe is inserted through a mouthguard into the esophagus and stomach, acquiring high-resolution images of retrocardiac structures under continuous vital sign and pulse oximetry monitoring.

  • Comprehensive Hemodynamic Profiling: Non-invasively calculates pulmonary artery pressures, stroke volume, systemic vascular resistance estimates, and intracardiac filling pressures.

RECOVERY

For standard 2D and 3D transthoracic echocardiography (TTE), there is zero physical recovery time; patients can immediately resume driving, work, physical exercise, and normal diet. For transesophageal echocardiography (TEE), recovery involves a structured post-sedation protocol. Patients remain in a recovery unit for 60 to 120 minutes until sedation agents clear and protective airway reflexes fully return. Fasting continues until topical pharyngeal anesthesia resolves (typically 60–90 minutes post-procedure) to prevent aspiration. Mild throat soreness or irritation is common for 24 to 48 hours following TEE. Patients receiving sedation must arrange transportation home and refrain from operating machinery or making legal decisions for 24 hours.

WHAT WE TREAT

Echocardiography is used to evaluate and diagnose a wide spectrum of cardiovascular disorders, including:

  • Heart Failure: Evaluation of left ventricular ejection fraction (LVEF), diastolic dysfunction, and structural cardiac remodeling.
  • Valvular Heart Disease: Assessment of stenosis or regurgitation involving aortic, mitral, tricuspid, and pulmonary valves.
  • Cardiomyopathies: Differentiation of dilated, hypertrophic, restrictive, and stress-induced (Takotsubo) cardiomyopathies.
  • Ischemic Heart Disease: Identification of regional wall motion abnormalities during acute coronary syndromes or stress testing.
  • Congenital Heart Defects: Characterization of structural anomalies such as atrial septal defects (ASD), ventricular septal defects (VSD), and patent foramen ovale (PFO).
  • Pericardial Disease: Detection and quantification of pericardial effusion, cardiac tamponade, and constrictive pericarditis.
  • Embolic Sources: Detection of intracardiac thrombi, cardiac tumors (such as myxomas), and infective endocarditis vegetations.

PREPARATION

Standard transthoracic echocardiography (2D/3D TTE) requires no dietary restrictions, medication adjustments, or physical preparation. Patients should wear comfortable, two-piece clothing to facilitate chest exposure. For transesophageal echocardiography (TEE), strict fasting (NPO) for food and liquids is required for at least 6 hours prior to the exam to minimize aspiration risks. Patients must disclose bleeding disorders, swallowing difficulties, esophageal strictures, or previous upper gastrointestinal surgery. Anticoagulants and antiplatelet medications are reviewed by the clinical team, and arrangement for an adult driver is mandatory due to intravenous sedation.

RISKS

Transthoracic echocardiography (2D and 3D) is exceptionally safe with no documented direct physical complications; rare minor discomfort may occur from local transducer pressure on the chest wall. Transesophageal echocardiography (TEE) carries minimal but distinct procedural risks due to probe insertion and intravenous sedation. Minor side effects include transient sore throat (10–15%), minor mucosal abrasion, and mild lip or tongue irritation. Rare serious complications include esophageal perforation (<0.01–0.03%), major gastrointestinal bleeding (<0.05%), aspiration pneumonia, transient laryngospasm, cardiac arrhythmias during insertion, or adverse reactions to sedative agents.

JOURNEY

The diagnostic journey for echocardiography begins with a clinical referral triggered by symptoms such as dyspnea, chest pain, palpitations, or murmurs discovered during examination. During the pre-procedure phase, patient evaluation determines whether standard 2D/3D transthoracic imaging or specialized transesophageal echocardiography is required. On the day of testing, standard transthoracic procedures require minimal preparation, while transesophageal exams require fasting and conscious sedation setup. The procedure involves transducer placement—either externally on the chest wall or internally within the esophagus—to acquire standard diagnostic views. Post-procedure recovery is immediate for transthoracic exams, whereas transesophageal procedures involve a 1–2 hour monitoring phase until sedation wears off. Final diagnostic reports are integrated into the patient's care plan to guide medical management, surgical referral, or lifestyle modifications.
