OVERVIEW

Sinus lift surgery belongs to the subspecialties of oral and maxillofacial surgery and periodontics. Its primary goal is to correct severe bone deficiencies in the posterior upper jaw, specifically the molar and premolar regions. When teeth in the upper back jaw are lost, the surrounding jawbone naturally dissolves over time, while the air-filled space above it—the maxillary sinus—expands downward in a process called sinus pneumatization.

This structural change leaves insufficient residual bone height to anchor dental implants safely. Sinus floor elevation raises the thin mucous membrane lining the sinus cavity (the Schneiderian membrane) and fills the newly created space with specialized bone graft material. Over several months, this material integrates with the patient's biological bone, generating a dense foundation capable of supporting permanent implant-supported dental restorations under functional chewing loads.

PROCEDURE

Sinus lift surgery involves a structured clinical protocol performed under strict aseptic conditions. The patient receives local anesthesia, occasionally supplemented with intravenous conscious sedation. For the traditional lateral window approach, an incision is made along the posterior upper gum line to elevate a full-thickness mucoperiosteal flap, exposing the outer wall of the maxilla. Using a surgical handpiece or piezosurgical device, the surgeon cuts a small rectangular osteotomy window into the outer bony wall of the sinus. The underlying Schneiderian membrane is meticulously detached and lifted upward using specialized mucosal elevators, creating a void along the sinus floor. Bone graft material (autograft, allograft, xenograft, or synthetic ceramic) is carefully packed into this newly created sub-membranous space. A protective resorbable collagen barrier membrane is placed over the lateral bone window, and the soft-tissue flap is repositioned and tightly sutured with non-resorbable or slow-resorbable sutures. In the crestal approach, access is achieved directly through the future implant site on the alveolar ridge using calibrated osteotomes or specialized drills to gently tap and elevate the sinus floor internally.

RECOVERY

  • Days 1–3: Expect mild to moderate local swelling, minor oral oozing, and temporary nasal congestion. Pain is typically manageable with prescribed non-steroidal anti-inflammatory drugs (NSAIDs) or mild analgesics. Activity should be limited, and nose blowing must be avoided.
  • Days 4–7: Facial edema peaks around day 3 and gradually subsides. Patients resume soft food diets and light non-strenuous daily activities.
  • Days 10–14: Intraoral sutures are evaluated and removed by the surgical team. Soft tissues generally achieve preliminary mucosal closure.
  • Months 1–3: Early mineralized matrix deposition occurs within the graft. Biological healing progresses internally without visible external symptoms.
  • Months 4–9: Complete osteoclastic and osteoblastic remodeling yields dense host bone integration, allowing for definitive dental implant placement or functional loading.

  • Posterior Maxillary Atrophy: Severe loss of alveolar bone height following single or multiple upper molar and premolar tooth extractions.

PREPARATION

Pre-operative preparation begins with a detailed clinical history, physical examination, and diagnostic imaging. A high-resolution cone-beam computed tomography (CBCT) scan is essential to assess 3D bone dimensions, detect sinus pathology, evaluate mucosal thickness, and rule out ostium obstruction. Patients with active sinus inflammation or allergic rhinitis are pre-treated or referred to an otolaryngologist prior to surgery. Pre-surgical instructions typically mandate prophylactic oral antibiotics (e.g., amoxicillin or clindamycin) starting 24 hours prior to the procedure, chlorhexidine antiseptic oral rinses, and prescription nasal decongestants to minimize sinus pressure. Patients undergoing sedation must observe strict fasts for 6–8 hours prior to surgery and arrange for adult escort transportation post-procedure.

RISKS

Complications following sinus lift surgery are generally manageable when identified promptly. The most frequent intraoperative complication is Schneiderian membrane perforation, occurring in approximately 10% to 20% of cases; small tears are managed immediately using resorbable collagen membranes or suture repair, whereas large tears may necessitate aborting the procedure to allow healing. Post-operative risks include graft infection, acute or chronic maxillary sinusitis, graft displacement, wound dehiscence, hematoma formation, and persistent oral-antral fistula (an abnormal passage between the mouth and sinus). In rare instances, partial or complete failure of graft integration occurs, requiring complete graft removal and secondary revision surgery after several months of soft-tissue resolution.
