OVERVIEW

The primary goal of varicose vein treatment is the elimination of lower extremity superficial venous reflux (abnormal backward blood flow in leg veins near the surface). When internal micro-valves inside leg veins fail, gravitational forces cause blood to pool, generating high hydrostatic pressure. Modern vascular medicine uses catheter-based heat or chemical agents to collapse diseased vessel walls. This approach re-routes venous circulation into functional deep veins without requiring open surgical removal.

PROCEDURE

Under real-time ultrasound monitoring, the target leg vein is mapped and marked. The skin is sterilely prepared and numbed with local anesthetic. A small needle puncture provides access to the vessel, allowing insertion of a micro-guidewire and a thin sheath. An endovenous thermal catheter (laser fiber or radiofrequency element) or chemical sclerosant catheter is advanced to the target position, strictly maintained at least 2.0 cm away from the deep vein junction (saphenofemoral or saphenopopliteal junction). For thermal techniques, tumescent local anesthesia is injected under ultrasound guidance into the tissue surrounding the vein to insulate adjacent nerves and skin while compressing the vessel around the device. Energy is applied as the catheter is steadily withdrawn, thermally coagulating the vein wall. For sclerotherapy, chemical liquid or foam is injected directly into smaller or tortuous vein segments. The puncture site is covered with a small dressing, and a compression bandage or stocking is immediately applied.

RECOVERY

Recovery following endovenous ablation or sclerotherapy is characteristically rapid and predictable. Patients are instructed to walk for 15 to 30 minutes immediately after the intervention to maintain deep venous blood flow and prevent blood clots.

During the first 48 hours, mild localized bruising, tightness, and mild tenderness along the treated vein segment are expected. Prescription analgesics are rarely necessary; over-the-counter non-steroidal anti-inflammatory drugs (NSAIDs) or paracetamol generally provide adequate pain control. Compression stockings (20–30 mmHg) are typically worn continuously for the first 24 to 48 hours, followed by daytime wear for one to two weeks depending on initial vein diameter and protocol.

Normal walking, light household activity, and office desk work can be resumed immediately or the following day. High-impact aerobic exercise, heavy weightlifting, hot baths, saunas, and long-distance air travel should be avoided for two weeks post-procedure to ensure vessel closure and minimize inflammatory complications.

WHAT WE TREAT

Varicose vein treatments address primary and recurrent venous insufficiency (impaired blood return to the heart) and its secondary clinical manifestations, including:

  • Symptomatic varicose veins causing heaviness, aching, throbbing, or nocturnal leg cramps.
  • Chronic venous insufficiency (CVI) accompanied by lower leg edema (fluid retention and swelling).
  • Stasis dermatitis (inflammatory skin reddening and eczema caused by pooled blood).
  • Lipodermatosclerosis (induration, darkening, and hardening of subcutaneous fat and skin above the ankle).
  • Venous stasis ulcers (chronic skin breakdown and open wounds near the malleolus).
  • Symptomatic spider veins (telangiectasias) and reticular feeder veins causing localized discomfort or burning.
  • Bleeding varicose veins resulting from thin-walled, high-pressure superficial veins near the skin surface.

PREPARATION

Pre-treatment preparation begins with a detailed diagnostic duplex ultrasound scan performed by a qualified vascular specialist. Patients must inform their clinical team of all current medications, particularly blood thinners (anticoagulants or antiplatelet agents), and any history of deep vein thrombosis (DVT) or clotting disorders. On the morning of the procedure, legs should be clean and free of lotions, creams, or oils. Comfortable, loose-fitting trousers or skirt and flat footwear should be worn to accommodate post-procedure compression dressings. Patients are advised to remain well-hydrated prior to arrival, as hydration improves peripheral vein access. Fasting is generally not required unless conscious sedation is specifically planned.

RISKS

Complications following modern varicose vein treatments are uncommon and predominantly mild. Minor, temporary side effects include localized skin bruising, temporary numbness or altered sensation (paresthesia) along the calf or thigh from cutaneous nerve irritation, localized hyperpigmentation (skin darkening), and mild superficial thrombophlebitis (sterile inflammation of a treated vein segment). Less common risks include skin thermal burns, localized wound infection, and incomplete vessel occlusion requiring secondary treatment. Rare but serious complications include deep vein thrombosis (DVT) occurring in less than 1% to 2% of cases, pulmonary embolism (PE), severe allergic reaction to sclerosant agents or local anesthetics, and arterial puncture. Long-term risks include deep vein extension or vessel recanalization (re-opening of the sealed vein) over time.

Pre-Procedure Evaluation

Patients undergo a detailed physical examination and formal duplex ultrasound imaging (a diagnostic sound-wave scan assessing blood velocity and vessel structure). This mapping identifies specific points of valvular incompetence, measuring vein diameters and depth to select the optimal treatment technique.

Procedure Phase

Treatments occur in an outpatient setting under local or tumescent anesthesia. Using direct real-time ultrasound guidance, the vascular specialist inserts a micro-catheter or fine needle into the target vein. Thermal energy (EVLA or RFA) or a sclerosant chemical foam is applied sequentially, causing immediate vessel collapse and closure.

Post-Procedure Recovery

Patients walk immediately following the procedure to promote deep vein circulation and lower clotting risks. Medical-grade compression stockings are worn continuously for several days. Full return to normal light activity occurs within 24 to 48 hours, while strenuous exertion is postponed for one to two weeks.

Long-Term Follow-Up

A follow-up duplex ultrasound is performed within one to two weeks to confirm complete vein occlusion and verify that the deep venous system remains clear. Long-term monitoring evaluates clinical symptom relief and monitors for recurrent varicose veins over subsequent years.
