Typical hospital stay after VATS lobectomy is roughly 4-5 days (median around 4 days in multiple series including the VIOLET trial)5. The chest drain is usually removed in 2-4 days. Pain is controlled on oral medications within the first few days. Discharge happens once the patient can walk short distances independently. Compare this with open thoracotomy, where the typical stay is 6-8 days. The actual length depends on how quickly the air leak settles, pain control, and whether there are any complications.
Different, not worse, for the cancer itself. The VIOLET trial directly compared the two in 503 patients and found equivalent oncological outcomes at 1 year (recurrence 7.7% VATS vs 8.1% open). Both approaches treat the cancer effectively when the anatomy is suitable. Where they differ is the recovery experience: VATS has less in-hospital pain, fewer complications, shorter hospital stay, and faster return of physical function. If your surgery is planned as open thoracotomy, that is almost always because of tumour anatomy (location, size, structures involved). Not because the operation is less effective.
Typically roughly 2-4 weeks after VATS, and 4-6 weeks after open thoracotomy. The clearance depends on three things. Pain controlled enough for you to perform an emergency stop without hesitation. Chest-wall healing far enough along that steering is not painful. And your motor-insurance policy terms (some policies have specific post-surgery exclusions). Confirm the exact restriction with your surgical team before driving. This is not optional guidance.
No, and if anything, VATS may be slightly better. The VIOLET trial found equivalent 1-year oncological outcomes. A 2026 Lancet individual-patient-data meta-analysis of 1,185 patients across randomised trials went further. It showed a 21% overall survival benefit for VATS lobectomy (pooled hazard ratio 0.79, 95% CI 0.65-0.96) compared with open surgery. The authors attributed this partly to lower short-term complication rates with VATS. The practical message: when the anatomy allows VATS, that is now the preferred approach; when it requires open thoracotomy, long-term survival is still strong.
